Virtual Enrollment Form

By signing this form below, I consent to my child’s registration for and participation in the Novi Community School District’s Novi Virtual courses and understand that the collected information is needed for both registration and state and federal reporting.

This consent covers all forms of distance learning courses including, but not limited to: satellite courses, video courses, and web-based courses. I understand and consent to my child’s image being transmitted during video portions of distance learning and online courses.

The District has allocated funding based upon enrollment commitment in Novi Virtual. This funding includes, but is not limited to, staff and resource allocations. I understand that my election for virtual instruction is made for the entire school year, unless otherwise required by law, an IEP or 504 plan, or other extenuating circumstances as determined by Novi Community School District.

I understand that virtual learning requires self-discipline and may not be appropriate for all students. Students in Novi Virtual must:

Be self-motivated and follow time deadlines.
Be able to work independently.
Be proficient in using technology or have assistance at home (parent/guardian).
Have access to reliable internet. (Contact Technology if you need assistance.)
Verify class attendance by turning their video conferencing camera on during synchronous instruction.
Adhere to the NCSD Handbook and Code of Ethics and the Acceptable Internet Use policies available on the district website.
Login each school day to the online course and participate in the weekly 2-way interaction assignments.
Attend required in-person assessments (class, district, state, etc.) in the windows provided.
Provide their own transportation to and from in-person assessments.
Ensure that communication is provided to the Novi Virtual office when the student is ill or will be out of the state or otherwise unavailable for an extended period of time.
If a High School student is scheduled into Michigan Virtual (MiVHS) coursework, adhere to the course schedules as prescribed, which may not be aligned with Novi Virtual semesters. Further, the MiVHS courses will show up as MiVHS on the official transcript and calculated as part of the GPA.

High School and Middle School students who are interested in post-secondary athletics are responsible for using the NCAA portal to confirm that all in person and virtual course selections meet NCAA eligibility guidelines.
